“Human Error” is widely accepted as the number one cause of serious unplanned events. As a result, organizations continue to expend valuable time, money and resource on corrective actions designed to better manage “Human Error” with the aim of preventing future, similar unplanned events from occurring. Unfortunately, the vast majority of such corrective actions simply miss the mark and fail to add meaningful value.
